Discussion：
日本を含む12か国で新規にSGLT1阻害薬（ダパグリフロジン）あるいはDPP-4 阻害薬を開始された２型糖尿病患者の国別傾向スコアマッチングによるネットワークメタ解析（181,620名ずつの対比較）において，12か国をプールした解析では全死亡，心不全による入院，心筋梗塞および脳卒中のリスクがSGLT1阻害薬で有意に低かった．ただし日本人のみの解析（19,752名ずつの対比較）では，発症イベント数が少なかった結果，全死亡および，全死亡と心不全による入院をプールした転帰に対してのみ，SGLT1阻害薬の優位性が示された．

- 高齢者ではビタミン B 12 の低下のリスクがもともと高い。
- ビタミン B 12 の代謝変化をきたすメトホルミン内服でさらに低下するリスクがある。
- ビタミン B 12 欠乏症は容易に加療できる疾患であり、メトホルミンを開始する前、開始後 1 - 2 年でスクリーニングし始めてよい可能性が高い。
