- 2型糖尿病のCKD患者において、メトホルミン投与群では、全死亡、末期腎不全への進展リスクが優位に低下していた。
- 本コホート研究の結果は、CKD患者へのメトホルミン投与を支持するが、無作為化試験による検証が望まれる。

20歳から70歳未満の日本人2型糖尿病で新規にDPP4iもしくはBG投与を開始された者を対象とし、HbA1cと医療コストを投与前と2年後で比較した。DPP4i群は1034名、BGは365名で、両群でHbA1cの改善に差はなかったが、年間医療費はBG群で有意に低コストであった。

Discussion：
成人 2 型糖尿病患者において、SGLT 2 阻害薬の使用は、GLP-1 受容体作動薬の使用と比較して痛風の発症率が低下した。以上より、SGLT 2 阻害薬が痛風のリスクを低下させる可能性が示唆された。

- SGLT 2 阻害薬を投与した患者に占める 1 型糖尿病患者の割合は研究対象全体としては低かったが、若年者で高かった。
- 1 型糖尿病に SGLT 2 阻害薬を適応外使用した患者の中で、特に 45 歳未満の女性の DKA リスクが顕著だった。
