2型糖尿病患者を対象とした大規模な国際的リアルワールド試験において，SGLT2阻害薬は他の糖尿病薬と比較し腎機能の低下速度が遅く，主要な腎イベントのリスクが低いことが示された．

阻害薬あるいはDPP-4阻害薬を新規に処方された2型糖尿病患者各28,877名を対象とした，傾向スコアマッチング・コホート研究である．SGLT2阻害薬はDPP4阻害薬と比較し，腎代替療法の開始など重篤な腎アウトカムが有意に少なかった．

2 型糖尿病患者では，若年 1 型糖尿病患者と異なり，SGLT2 阻害薬による糸球体内圧減少効果は，輸入細動脈収縮より，輸出細動脈拡張の影響が大きい可能性が示唆された．

- 2型糖尿病のCKD患者において、メトホルミン投与群では、全死亡、末期腎不全への進展リスクが優位に低下していた。
- 本コホート研究の結果は、CKD患者へのメトホルミン投与を支持するが、無作為化試験による検証が望まれる。

Discussion：
20歳から70歳未満の日本人2型糖尿病で新規にDPP4iもしくはBG投与を開始された者を対象とし、HbA1cと医療コストを投与前と2年後で比較した。DPP4i群は1034名、BGは365名で、両群でHbA1cの改善に差はなかったが、年間医療費はBG群で有意に低コストであった。
