Discussion：
胃バイパス手術は1年目と2年で3エンドポイント達成率を改善した。この効果持続性が重要となり、本結果で胃バイパス手術は3年目でも有意に複合エンドポイント達成を改善したが、臨床効果は3年で両群ともに減弱した。バイパス術患者でHbA1c7%未満は1年目73％から58％まで低下した。治療効果減弱の原因を糖尿病羅病期間、C-peptide、HbA1c、BMIから予測できなかった。

過去に言われていた血糖コントロール(HbA1c1%未満)と視力予後に関連があるという示唆との相違の原因・RISEandRIDE試験では著しい血糖コントロール不良例(HbA1c12%以上)は除外されている。
・糖尿病では初期のデータが微小血管異常に反映されてくるのは慢性期になってからであり、24ヶ月の経過にはあらわれていない。

遺伝的背景の等しくてもBMIが高いことは糖尿病のリスクになる。BMIと心筋梗塞、死亡率には関連は見られなかった。生活習慣介入により減量を促進させることは心血管疾患よりも糖尿病のリスク軽減につながると考えられる。
