- RAS 阻害薬開始後の 10 % 以上のクレアチニンの急激な増加は、死亡・心筋梗塞・心不全および末期腎不全の発症リスクの増加と強く関連していた。
- RAS 阻害薬開始前後のクレアチニンの推移を監視することで、その後の有害転帰のリスクが高い患者を特定することができるかもしれない。

Discussion：
実臨床において、SGLT2 阻害薬はDPP-4 阻害薬および GLP-1 アナログ製剤と比し、重症尿路感染症、および外来加療が可能な尿路感染症のリスクを増加させない。その点については安心して使用できる薬剤である。

- SGLT 2 阻害薬は重症、軽症とわず尿路感染との関連はなかった。

今回の study において、ACEIs と比較し ARB は心筋梗塞の既往がある高齢患者において有害であるどころか寧ろ優れた予後改善効果を示した。

DPP4 阻害剤の使用と比較して SGLT 2 阻害剤の使用による高齢糖尿病患者の性器真菌感染症のリスクは男女問わず増加を認めた。一方尿路感染症のリスク増加は認めなかった。
