尿アルブミン30 mg/g Cr未満の腎機能低下患者においても， 尿アルブミン高値は後の腎イベントの危険因子であった．これらのoutcomeを改善することができるか， その際の尿アルブミンの目標値設定が今後の課題．

Discussion：
型糖尿病を有する成人の大規模コホートにおいて，KDIGO分類に基づく
CKD有病率は18.5%であり，eGFR 60未満の27.4％が正常アルブミン尿であった．
死亡率を含むすべての腎・心血管アウトカムの発生率はeGFRが低下するにつれて徐々に
増加した．G1A1をreferenceとした場合，G2A1の群においてもHRは上昇した．
従来非CKDで低リスクと考えられてきたG2A1でも主要アウトカムのリスクが高く，早期
の段階で専門医の関与が必要と考えられた．糸球体過剰濾過を示す可能性があるGFR 105
以上の群では正常アルブミン尿の群ではすべてのアウトカムのリスク上昇につながらなか
った．アルブミン尿が増加すると腎アウトカムHRは上昇したが，MACE・全死亡のHRは

Discussion：
後ろ向きコホート研究により、緩やかな血糖コントロールを達成した高齢の2型糖尿病患者は、厳格な血糖コントロールを達成した高齢の2型糖尿病と比較し、ほとんどの感染症による入院リスクは有意に高くないことが示唆された。
