・非糖尿病肥満患者は Roux-en-Y 胃バイパス術によりインスリン代謝は健常人と同程度まで改善し、糖負荷に対する過剰反応は改善していたが、糖尿病肥満患者ではインスリン代謝や β 細胞機能は改善するものの健常人のレベルには達しなかった。

2 型糖尿病の寛解のためのプライマリケア主導体重管理を an open-label,cluster-randomised trial で行った。強化療法群は平均 10kg 体重減量し、12 か月で参加者のほぼ半数が糖尿病治療薬なしに非糖尿病状態への寛解を達成した。2 型糖尿病の寛解はプライマリケアの実践的な目標である。

・肥満合併 2 型糖尿病患者を対象に行った本研究において、生活習慣の介入＋医療管理と比較して、胃バイパス術を追加した群では、追跡 5 年で複合エンドポイントの達成率は有意に高いことを維持していた。・しかしその効果の大きさは 5 年間で減少してきていたため、その耐久性を評価するためには、さらなるフォローアップが必要である。

・個々の患者について、糖尿病リスクが中程度に上昇することは、一方でスタチン治療によって、心筋梗塞、脳卒中および心臓血管死が非常に有意に減少することとバランスをとれることが必要である。・CVD 予防のためのスタチンを処方されている糖尿病高リスク患者は、より慎重に血糖を管理し、健康的な生活習慣の行動を強化する必要がある。

Discussion：
重症肥満患者で肥満外科手術をうけた患者は疼痛、身体機能、歩行時間が Baseline と比較して大幅な改善がみられる。しかし、術後 1 年から 3 年目を比較すると改善率は低下していた。
