Discussion：
・IVRがDMEのみならず、PDRそのものの治療に有効である。と言うことを示した貴重なデータ、研究。・但し、当文献の本文はDiscussionを含め、IVセンティスの有用性を主張することに偏重する嫌いがある。
・日本で行われているPRPはより選択的かつ低侵襲であり、効果の持続性や、費用対効果の観点からもIVR継続療法はPRPと完全に取って代わるものでは未だないと感じた。
・本文では全身リスク不変と述べられているが、Supplemental contentのTable8、Carcliac clisordersはIVRで心疾患リスクの拡大を示している。

Discussion：
・高血糖・糖尿病の治療目標を十分に達成している高齢患者のうち、治療の脱強化が行われている割合が少ないことが分かった。・今後、脱強化の有無に伴う転帰の検討とともに、治療の脱強化を通じて過剰医療を減らすことに焦点を当てたガイドラインを作成する必要があるかもしれない。

Discussion：
メトホルミンは肥満妊婦から生まれる児の出生時体重に影響を与えなかった。今回のstudyで生まれた子供のさらなるフォローアップにより、将来的な長期的なoutcomeについて検証が必要である。現時点ではメトホルミンを肥満妊婦のpregnancy outcomeの改善目的に使用してはならない。
