Discussion：
出生時の低体重と不健康な生活習慣は相互に影響して、2型糖尿病リスクを高める。2型糖尿病の多くは健康な生活習慣により、予防可能であり、さらに出生時の低体重を引き起こす。要因に対する対策をとれば、羅患者は更に減らせる可能性が示された。また、低出生体重よりも不健康な生活の方が、発症に寄与する割合は高かった。

要旨1型糖尿病患者における胃壁細胞のプロトンポンプsubunit Aに対する自己抗体の陽性率は、年齢と共に上昇し、40歳以上で32.6％であり人種差も認められた。
HLAクラスII、TPO抗体、GAD抗体、21－OH抗体と有意な遺伝相関があり、継承率も高値であった。自己免疫疾患が重なる病態において重要な自己抗体の一つと考えられる。

Discussion：
・IVRがDMEのみならず、PDRそのものの治療に有効である。と言うことを示した貴重なデータ、研究。・但し、当文献の本文はDiscussionを含め、IVセンティスの有用性を主張することに偏重する嫌いがある。
・日本で行われているPRPはより選択的かつ低侵襲であり、効果の持続性や、費用対効果の観点からもIVR継続療法はPRPと完全に取って代わるものでは未だないと感じた。
・本文では全身リスク不変と述べられているが、Supplemental contentのTable8、Carcliac clisordersはIVRで心疾患リスクの拡大を示している。
