1型糖尿病におけるエンパグリフロジンの大規模臨床試験において、 日常診療のルーチンでの血中ケトン体検査から、将来の DKAを予測できるかを明らかにすることを目的とした。 1型糖尿病患者において、SGLT2阻害薬使用者の日常診療の最大ケトン体値 が0.8mmol/L以上である場合、翌月のDKA または重症ケトーシス発症リスクが3倍以上になることが明らかとなった。 定期測定でケトン体値が閾値を超えた場合には、インスリン量の 調整や教育入院のなどの介入を促す契機となり、今後1カ月間にDKAのリスクが高い患者を同定する有望な戦略となり得る。
