TrialNet Anti-CD3 Prevention Trial(NEJM.381：603, 2019)において、Oral minimal model (OMM)由来の指標(総インスリン分泌、IS(insulin sensitivity)、β細胞機能（処理指数 [DI]、φ total と IS の積）、インスリンクリアランス)を偽薬、実薬(CD3モノクローナル抗体：テプリツマブ)の両群間で検討した。OMM指標により、stage2の1型糖尿病に おけるインスリン分泌の不均一性を明らかにし、両群における急速進行型と緩徐進行型の 代謝表現型の違いを特定し得た。

Discussion：
型糖尿病を有する成人の大規模コホートにおいて，KDIGO分類に基づく
CKD有病率は18.5%であり，eGFR 60未満の27.4％が正常アルブミン尿であった．
死亡率を含むすべての腎・心血管アウトカムの発生率はeGFRが低下するにつれて徐々に
増加した．G1A1をreferenceとした場合，G2A1の群においてもHRは上昇した．
従来非CKDで低リスクと考えられてきたG2A1でも主要アウトカムのリスクが高く，早期
の段階で専門医の関与が必要と考えられた．糸球体過剰濾過を示す可能性があるGFR 105
以上の群では正常アルブミン尿の群ではすべてのアウトカムのリスク上昇につながらなか
った．アルブミン尿が増加すると腎アウトカムHRは上昇したが，MACE・全死亡のHRは

１型糖尿病をもつ成人を対象に① 糖尿病の知識と自己管理の方法を教育・指導する
、② 感情面に焦点をあててアプローチ、③ ①と②の統合プログラムの介入を行い、糖
尿病による負担とHbA1cへの効果を検討した。
すべての介入プログラムにおいて、糖尿病による負担、HbA1c共に有意な減少が見られた
。特に1型糖尿病をもつ成人を対象とした心理、感情に焦点をあてたプログラムが、糖尿
病による負担、HbA1cの両者において有効であった。
