Discussion：
2型糖尿病患者において、SGLT-2阻害薬とGLP-1受容体作動薬を新規開始し
たところ、 1年後の尿路感染症発症のリスクは両群で同等であった。 特に高血糖、UTIの
既往、過去の尿閉塞の既往があるような高リスク群でSGLT-2阻害薬使用群のUTIリスクは
低下していた。性器感染症についてはSGLT-2iの群でリスク上昇が確認され、特に治療初
期にその傾向が顕著であった。

TrialNet Anti-CD3 Prevention Trial(NEJM.381：603, 2019)において、Oral minimal model (OMM)由来の指標(総インスリン分泌、IS(insulin sensitivity)、β細胞機能（処理指数 [DI]、φ total と IS の積）、インスリンクリアランス)を偽薬、実薬(CD3モノクローナル抗体：テプリツマブ)の両群間で検討した。OMM指標により、stage2の1型糖尿病に おけるインスリン分泌の不均一性を明らかにし、両群における急速進行型と緩徐進行型の 代謝表現型の違いを特定し得た。
